An exploration of the UK unpaid carer's world

Triangle of Care update 8 Nov 2015    was here

I have today seen Appendix 1: Triangle of Care self-assessment tool on page 22 here and will very soon be finding out from 2g if anyone has heard of it.

It includes this text on the next page and my answers follow.

Action plan   -  Evidence of achievement

By whom?  Very good question - sad that nobody knows.

By when?   ditto

The main problem is that there is no evidence of achievement.  After all is said and very little is done, 2g only recognised the triangle in March 2015 and it has been available since 2010.

The main carer or carers are routinely identified and their views sought within the assessment process to help inform care.  (This is Comic Sans  text - Sans  = without evidence.)

Only when two dozen webpages have been presented.  Only when the carer has reached breaking point.

When the main carer or carers are routinely identified, it is thought best to tell them they can't be involved in the triangle via a third party.

An assessment of carer needs is carried out.   Only when two dozen webpages have been presented.  Only when the carer has reached breaking point.

Processes are in place to ensure that information is shared with all practitioners involved in care, including those

supporting other co-morbid conditions, to avoid conflicting treatments.

A record is made of lasting power of attorney(ies) and whether an advance plan and/or directive is in place.

People with dementia and their carers are regularly updated and involved with care plans and treatment which

focus on all their needs.

Only when two dozen webpages have been presented.

Only when the carer has reached breaking point.

When the main carer or carers are routinely identified, it is thought best to tell them they can't be involved in the triangle via a third party.

Only when the carer has reached boiling point.
