1  ADS aspects of the DMC project
  1. H of R claim 1 - I completed the two questionnaire without ref to SE - untrue

  2. Claim 2 - I wanted to change the evaluation system - untrue

  3. My request - provide evidence

  4. This was met with "I don't know what you are looking for.".

2   L.. ..  D M C- operation

Unacknowledged H & S comment: 
  1. All concerned prepare sandwiches and salads without rubber gloves

  2.                             are in and out of the small kitchen in proportionately too large numbers

  3. H & S aspects worsened by the lack of a door-stop on a strong-springed door

  4. Sessions are being operated involving brass instruments within which no components are cleansed before passing from person to person.  AFH had included disinfected wipes into previous comment on birthday cake-making and again in a music session.   Thorough risk assessment strongly advised on 31 August.

  5. Possibly  no recognition of the recent problem with bagpipe lung disease.  (Notified on 31 August and with further sessions programmed.)  AFH plans to avoid attending on such days.

  6. Unsupervised carees visiting the toilet present subsequent users with risk.  Men leave pools of urine on the floor.   Teresa follows with slippers.  AFH has cleaned mess without cleaning equipment, disinfectant and available rubber gloves.   Notified on 31 August and informed Leom DMC that I have provided cleaning equipment. The reply was that such equipment had not been approved at the start of the project and the manager would consider the situation.

  7. Unsupervised carees visiting the toilet present other users with mutual embarrassment.  Some carees do not lock the door.  AFH provided Engaged/Vacant signage on 31 August.   The reply was that the manager would consider the situation.

  8. There has been no hot water, soap, nailbrushes etc in the toilets.  Disinfected wipes have appeared recently.

  9. All this bears heavily upon "All concerned prepare sandwiches and salads without rubber gloves.".
